E-rate Form 479 and Letter of Agency Submissions

Each year MOREnet seeks E-rate discounts on Internet access and other services on behalf of eligible MOREnet members. Even if you do not apply for other E-rate services such as local and long distance phone service as an organization, you must complete the Form 479 and Letter of Agency to MOREnet. If you do apply for other E-rate services, this does not affect your ability to do so.

Unlike previous years, this year's Letter of Agency MUST BE ON YOUR ORGANIZATIONAL LETTERHEAD. You will note that the letter is identical to previous years, but because of new SLD policy, we must have your Letter of Agency printed on your organizational letterhead.
